IV HODGKIN LYMPHOMA
Findings: On the front plate, it highlights a widening of the sus-hilaire mediastinum right with erasure of the straight para-tracheale Strip making suspect a ganglionic mass. On chest CT, confirmed a casting of paratracheales Lymphadenopathy straight retro-cellars. Moreover, at the level of the upper abdomen, presence of multiple hypodenses liver damage and a splenic hypodense lesion, indicating a subdiaphragmatic achievement of lymphoma.
Diagnosis: IV Hodgkin Lymphoma
Discussion: Lymphomas are divided into Hodgkin's disease and non-hodgkiniens lymphoma. It is not possible to make the diagnosis of a disease and Hodgkin Lymphoma non-Hodgkin on the single radiological aspect of the thorax, as these two groups of lymphomas give the same radiological signs. The only difference is the variable frequency of the headquarters of Mediastinal and lung damage according to the type of lymphoma. This is the clinic and the pathology that will allow the distinction. According to the classification of Rye, there are 3 or 4 histological types with different prognostic factors (type 1: Lymphocytic predominance; type 2: Nodular sclerosis; type 3: mixed cellularity; type 4: Lymphocytic depletion which is actually) probably a non Hodgkin Lymphoma). The Ann Arbor classification allows to assess the spread of the disease in 4 stages depending on whether one or more lymph node groups are suffering from hand and side of the diaphragm (stage I: a ganglionic group; stage II: two lymph node groups on the same) side of the diaphragm; stage III: reached ganglionic on both sides of the diaphragm; stage IV: infringement of an organ).
